Every withdrawal at your ATM passes through a processor. Here's what happens in the few seconds between a card going in and cash coming out.
They insert a card at your ATM and enter an amount.
The processor routes it through the card networks to the customer's bank, which approves or declines it.
Approved withdrawals, plus any surcharge due to you, are settled to the bank account set up for the machine, under your processing agreement.
